The sore spot gives the first answer
The first steps out of bed may feel stiff. The ache may sit deep in the groin or on the bony outside.
Those two spots don't mean the same thing. A deep ache often begins in the joint, while outside soreness may come from nearby muscle cords.
Deep groin soreness often starts in the joint
Your hip may loosen after several calm steps. Socks, shoes, stairs, and rising from a low chair can still hurt.
Turning your leg may bring a sharp catch. Arthritis may be involved when rest eases the deep ache.
One sore move can't settle the cause. An exam checks how the hip turns, where it hurts, and whether the walk has changed.
A sore tendon can hurt on the bony outside
The word tendon means firm tissue that anchors muscle onto bone. Several tendons run from the buttock muscles to the bony side of the hip.
That spot may hurt most under your weight at night. Crossed legs or leaning onto one hip can press it too.
Complete rest usually isn't enough. A doctor or physical therapist can choose a safe strength move, such as slow rises from a firm chair.
Keep both hands close for support and stop if the move feels sharp. If the next morning hurts more, the exercise was too hard.
An exam can separate joint soreness from outside soreness
Book a regular exam when soreness keeps breaking sleep or changing errands. Bring an old X-ray if available, but don't delay care to get one.
Call the doctor that day if fever comes while the hip is hot and swollen. Don't wait until tomorrow.
After a fall, use urgent care or the emergency room when you can't put weight on that leg. The emergency room is safer if standing feels impossible.
Until then, take easy steps on level ground and don't lie on the aching side. Place a pillow so the knees stay apart while resting.

In a three-arm randomized trial of 204 people with MRI-confirmed gluteal tendinopathy, an eight-week physiotherapist-led education and exercise programme produced success on global rating of change in 51/66 participants at 8 weeks versus 38/65 for a single corticosteroid injection and 20/68 for wait-and-see. Education plus exercise beat the injection at 8 weeks (risk difference 19.9%) and still beat it at 52 weeks (20.4%).

A 2025 systematic review restricted to medium/high-quality low-risk-of-bias RCTs found only four interventions with demonstrated efficacy in gluteal tendinopathy: exercise and education (moderate strength, medium short-term effect on pain SMD 0.95 and function SMD 0.91), corticosteroid injection (small short-term pain effect SMD 0.51), PRP (superior to corticosteroid for short-term function), and focused shockwave therapy (long-term pain superiority over corticosteroid). Exercise and education is the recommended core.

A Bayesian network meta-analysis of 11 randomized trials (1353 patients) in hip osteoarthritis found that at 2-4 months and 6 months NO injectable - corticosteroid, hyaluronic acid or platelet-rich plasma - significantly outperformed a saline placebo injection for pain or function. Pooled change from baseline exceeded the minimal clinically important difference in every arm including placebo.

OARSI 2019 designates arthritis education plus structured land-based exercise as CORE treatments for hip OA, and explicitly states that intra-articular corticosteroids, intra-articular hyaluronic acid and aquatic exercise - Level 1B/2 treatments for KNEE OA - were NOT recommended for individuals with hip or polyarticular OA. Oral and transdermal opioids are strongly not recommended (Level 5).
